Income 

  • Employer Jobs: Form W-2s for every job worked in 2019
  • Independent Contractor: 1099 and itemized list of your expenses
  • Unemployment: Form 1099-G
  • Gambling Winnings: Form 1099-G 

Business Owners

  • Independent Contractor: 1099 and itemized list of your expenses (Go to 'Expense Sheets' tab and download Schedule C and Business Car Expense)

Education

  • Student Loans: Form 1098-E
  • University/College: Form 1098-T, books, parking, and supplies
  • Ask parents if they will be claiming you. If yes, give Form 1098-T to parent. If no, ask how many years have they taken the American Opportunity Credit on your behalf. 

Dependents

  • Newborn: His/her social security card
  • Dependents: Proof of residency (Must be from doctor, school or Form 1095 from health insurance)
  • Daycare: Federal ID number, name, address, phone number, and amount 
  • Paid Family Leave: Form 1099-G

Home Owners

  • Home Owner: 1098 mortgage interest statements and property taxes 
  • Bought, Sold, or Refinanced Home: Final closing statement from escrow
  • Bought Car: Purchase/lease agreement (long piece of paper)
  • Car Registrations

Investments

  • Stock Sales: 1099-B
  • Interest Income: 1099-I
  • Dividend Income: 1099-DIV
  • 401K/IRA Cashout: Form 1099-R
  • Pension Distribution: Form 1099-R
  • HSA Distribution: Form 1099-SA
  • HSA Contribution: Form 5498-SA

Health Care

  • Covered California: Form 1095-A (MANDATORY to file)
